Improvement Stories

Self-Management Support

IHI.org has two types of Improvement Stories:

  • IHI.org Stories that describe the changes and results in specific organizations.
  • Improvement Project Reports from IHI.org users that describe improvement projects in their organizations.

 

We all learn from others' experiences testing and implementing changes in real settings — who should be on the team; what measures were tracked; which changes worked best or didn't work at all; and what lessons were learned.

UNITE HERE Health Center (New York, New York, USA) redesigned care for low-wage, immigrant patients with obesity by promoting self-management support as the foundation of the patient care plan, created patient registries to document collaborative goals, and spread these changes to address other chronic conditions within the medical home.

Self-Care as a Means to Increasing Capacity and Patient Choice in Haemodialysis Units
The Modernisation Initiative (London, England), in partnership with several renal centres, introduced a self-care programme for haemodialysis (HD) patients that resulted in increased capacity and greater patient choice in terms of treatment options, with almost 10 percent of all HD patients now fully independent and a further 50 percent achieving partial independence.
