This comprehensive website includes information on diabetes as well as self-management. Presently there are active Message Boards to share ideas and opinions on a variety of topics with people affected by diabetes. In addition, the Diabetes Learning Center promotes better diabetes self-management in the sections entitled Nutrition, Exercise, and Weight Loss. The Virtual Grocery Store focuses on healthy food choices and meal planning. Much of the information is offered in Spanish.

Self-Management / Adherence
The goal of this Health Research and Educational Trust project, funded by The Robert Wood Johnson Foundation, was to help health care organizations create effective, culturally sensitive self-management support programs. For eight months, teams from six provider organizations tested, adapted and implemented evidence-based self-management support principles in their settings to produce improvement at both the individual patient level and the practice level to maximize positive health behaviors, satisfaction with health care, and quality of life for their patients with chronic conditions.
