Setting Aims

Model for Improvement The first step in improving medication safety is making a solid commitment to reduce harm. This commitment should be reflected in a strong and well-worded aim statement that sets an aggressive goal. It is critical that the overall aim reflect a goal to reduce harm significantly, in a measurable way and in a specified time frame. For example, an organization wishing to make a real commitment to reducing harm involving medications should set an aim stating: Reduce adverse drug events across the organization by 75 percent within 1 year.

Senior leaders should be involved in setting the aims, to ensure that the aims are aligned with the organization’s strategic goals. When senior leaders approve the aims, they should also make a commitment to give the team whatever support is needed to achieve those aims.

Examples of Effective Aim Statements

The following aim statements are grouped according to the overall aim of reducing harm from medications as well as the four key areas, Culture, High-Hazard Medications, Core Medication Processes, and Reconciliation. Note that all of these aims are outcomes oriented, with specific numeric goals and timeframes. An organization typically has several teams working on more than one aim at a time, but they should all report to one oversight safety team.
 

Harm

  • Reduce adverse drug events (ADEs) in all medical and surgical units by 75 percent within 11 months.

Culture

  • Have at least 50 percent of staff surveyed reporting a positive safety culture within 1 year.

High-Hazard Medications

  • Reduce ADEs involving anticoagulants by 75 percent within 9 months.
  • Reduce the incidence of blood sugar < 50 by 75 percent within 8 months.
  • Reduce the incidence of over-sedation of ventilated patients by 50 percent within 9 months.
